    Registration and Application

    This 12-week contemporary street art workshop series will provide young people with an opportunity to take a leadership role in the Gaylord Area Council for the Arts Art in the Alley Project. Students will develop their art and design skills while also engaging in a collaborative leadership project. This series is open to all high school aged students who are Otsego County residents.

    In order to ensure your child receives all of the benefits from this program, please take a moment with your child to answer the following questions:

    Attendance policy

    An important aspect of the program is developing team leadership skills. As such it is important for your child to attend all of the scheduled classes.  We however, recognize that circumstances arise such as illness that may cause your child to miss a few classes.  We, therefore, allow for up to 3 excused absences.  More than 3 excused absences will result in your child being dropped from the program.  By checking the box below you agree to and understand this program policy. 

    Code of Conduct Policy

    One of the goals of this program is to develop leadership skills in a collaborative safe environment and your child understands that by participating in this program they are representing Rise and Gaylord Area Council for the Arts.  As such, students are expected to conduct themselves in a professional manner at all times during the program. If a student is found to have violated this standard, Rise/Gaylord Area Council for the Arts may terminate the student's participation in the program at anytime at their discretion. Additionally, it is the mission of RISE to maintain a drug free space for all of their programming and activities. The use of tobacco, nicotine, alcohol or other drugs is strictly prohibited by students participating in any RISE program. As such your child agrees to refrain from the use of these substances while participating in the program. By checking the box below you and your child acknowledge and agree to this policy. 

    Child image release

    New technology provides many more opportunities for students and their work to be showcased. RISE: Otsego Substance
    Free Youth and Gaylord Area Council for the Arts require your permission to use any photograph, digital images, slides, video, or writings involving your child. The use of photographs, voice, moving images, or writing images include but are not limited to RISE/Gaylord Area Council for the Arts' websites, Facebook, brochures, posters, theater ads or other forms of print electronic media.

    By checking the box below you authorize RISE: Otsego Substance Free Coalition, Gaylord Area Council for the Arts and RISE Youth to use photographs, digital images, slides, video, writings, or other similar media, including internet application in which your child appears and you also understand these images may be used for possible publication or shown for informational or education purposes to a variety of audiences. I also understand that this Child Image Release Form will remain in effect until I revoke it in writing in a letter addressed to RISE: Otsego Substance Free Coalition, 95 Livingston Blvd, Gaylord, MI 49735, or until the materials are no longer needed by RISE or Gaylord Area Council for the Arts. 

    Parental/Gaurdian permission

    By checking the box below you acknowledge you are the legal guardian of the student named on this application and that you give permission for this student to participate in the Art in The Alley-Rise Street Art Leadership Series. You also understand that permission is granted for the duration of the Art in The Alley-Rise Street Art Leadership Series and that you may withdraw your permission by contacting Ellen Templeton in person or in writing at the following address: 95 Livingston Blvd, Gaylord, MI 49735.
